Extract from Supplement to The Gazette no. 31043, dated 2nd December 1918, pg 14252:
Lt. Samuel Mayoh, I/5th Bn., N. Lan. R.,
T.F., attd. 170th L.T.M. By.
For conspicuous gallantry and devotion to
duty whilst leading a section of the battery
in support of infantry. He got his section
into action under heavy machine-gun fire
and cleared out three different enemy
machine-gun nests, thus enabling the advance to continue. When the infantry in
front consolidated he, having finished his
ammunition, formed his section and carriers
into posts and prepared to give assistance
with rifle and bayonet in the event of a
counter-attack. He held these positions
until relieved.
