The Dutch Submarine HNMS O19 accidentilly run aground on a reef in the South China Sea on the 8th July 1945 and was then scuttled after two days attemped rescue by USS Cod.
Name on the distinguished service medal is correct. The frame is sealed so extra images not possible but the name is definitely correct.
