Noting Guinness' Hampshire and London addresses and his other contact information, as well as his character name 'Ben Kenobi.' The rest of the sheet remains unfilled, presumably because Guinness had a very different contract to that of the other actors on production. A small blue tick features to the top right-hand corner.
